For example I had the Return to the keep on the borderlands placed in the southern mountains of yeomanry,*spoilers alert* to cut a long story short even though they defeated most the humanoids and defeated the cult in the caves they made a very big enemy of some locale dragons all over keeping the egg. The big caveat is win or lose any modules, adventure paths etc we place in the world whether completed or not must have a lasting effect and the next set of charecters will begin their adventure a year after the death or retirement of the last. To explain how i have always run my games, every 5-6 years we decide upon a setting and will then run all our characters and games in that setting.
